What's required for kids enrolling in school for the first time?
-Diphtheria, tetanus and whooping cough/Pertussis (DTP) vaccine, 5 doses.
-Poliomyelitis vaccine, 4 doses. If you're 18 and haven't had one, no vaccine is necessary.
-Measles (rubeola) vaccine, 2 doses. If you've had measles prior to Jan. 1, 1994, no vaccine is needed.
-Rubella vaccine, 1 dose.
-Mumps vaccine, 1 dose.
-Haemophilus influenzae b (Hib), 3 doses of HbOC or PRP-T or 2 doses of PRP-OMP, followed by a booster. If vaccinated on or after 12 months and before 15 months, you only need 2 doses. If vaccinated after 15 months, only 1 dose is needed. No one passed age 5 needs to be vaccinated.
-Hepatitis B, 3 doses.
-Varicella vaccine, 1 dose.
For sixth-graders:
-TDP booster is required for people entering sixth grade on or after Aug. 1, 2008.
Going to college?
-Tetanus/diphtheria toxoid, 3 doses, one within the last 10 years; if enrolling in college for the first time after July 1, 2008, you must also have a booster of DTP if a booster of tetanus/diphtheria toxoid or DTP vaccine hasn't been given in the last 10 years. DTP is not required for people over 64.
-If you've had measles prior to Jan. 1, 1994, or been born before 1957, no vaccine is needed.
-If you're 50 or older and have never had the rubella vaccine, none is needed except in outbreak situations.
-If you're going to college after your 30th birthday and before Feb. 1, 1989, the rubella vaccine is not required except in outbreak situations.
-If going to school or college prior to July 1, 2008, it's not required to receive a second dose of mumps vaccine; someone born before 1957 is not required to receive a mumps vaccine.
-If born before Oct. 1, 1988, you're not required to be vaccinated for Haemophilus influenzae b (Hib)
-If born before July 1, 1994, you're not required to be vaccinated against Hepatitis B.
-If born before April 1, 2001, you're not required to have the varicella vaccine.
